I/we acknowledge the contagious nature of the Coronavirus/COVID-19 and that the Public Health Authorities still recommend practicing social distancing. I/we further acknowledge that Farias Soccer Academy has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I/we further acknowledge that Farias Soccer Academy cannot guarantee that I/we will not become infected with the Coronavirus/COVID-19. I/we understand the risk of becoming exposed and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself/ourselves and others, including, but not limited to, staff, and other clients and their families of Farias Soccer Academy. I/we will not hold Farias Soccer Academy legally liable should I/we contract the Coronavirus/COVID-19 while attending sessions held by Farias Soccer Academy.

I/we voluntarily seek sessions provided by Farias Soccer Academy and acknowledge that I/we are increasing my risk of exposure to the Coronavirus/COVID-19. I/we acknowledge that I/we must comply with all set procedures to reduce the spread while attending my sessions. I/we acknowledge that we have not tested positive for the Coronavirus/COVID-19, and I/we acknowledge that I/we are not currently displaying symptoms of the Coronavirus/COVID-19.

Farias Soccer Academy
Liability & Media Release Waiver - REQUIRED

The individual signing this Liability and Media Release Waiver (“Waiver”) certifies that he/she is the Parent/Guardian/Player 18 Years of age or older ("Adult"), and accepts this Waiver on behalf of the Player named below for training, games, and events held by Farias Soccer Academy (“Academy”) and recognizes that there are risks of damage or injury arising from participation in these activities.
By his/her signature below or by participation in the training, games, and events, the Player and Adult voluntarily agrees to assume and/or incurs all risks of loss, impairment, damage or injury of any kind, including death, that may be sustained or suffered by participation whether or not the result in whole or in part of acts or omissions, negligence or other unintentional fault of the staff or owners. In addition, the participant (including his/her heirs, assigns and personal representatives) agrees to release, hold harmless, and indemnify the Farias Soccer Academy, Coaches and Owners (“Staff”) from and against any claims, demands, actions, causes of action, lawsuits, expenses, or losses (including attorneys’ fees) on account of property damage or personal injury (including death) arising out of or attributable to the individual’s travel to or participating in training, games, and events.
By his/her signature below or by participation in training, games, and events, the Player and Adult voluntarily waives all rights of ownership to any written, photographic or video use (whether in written or electronic form) of the participating individual’s name, image or representation (“Data”), and hereby grants permission for the publication, announcement and/or distribution of said Data thereof by the Farias Soccer Academy, Staff, and their Media Representatives.
This Assumption of Risk and Media Release Waiver applies to the Farias Soccer Academy and all of their owners, coaches, junior coaches, volunteer coaches, staff, advisors and/or representatives.
The undersigned acknowledges that he/she has read and understands this document and that no changes will be made to this document.
